Applicable to All Programs

• Request for Evaluation form (MS Word)
  Programs seeking accreditation must submit a Request for Evaluation form. To secure an ABET evaluation visit in the fall, this form must be submitted to ABET by January 31 along with one graduate's official original transcript from the most recent graduating class for each program requesting evaluation.

• Request for Approval form (for non-U.S. programs only) (MS Word)
  Institutions outside of the U.S. are also required to provide a Request for Approval form. To be eligible for ABET accreditation, this form must be completed by each appropriate recognition agency and submitted to ABET by January 31.

    • Describe the process used for establishing and revising any student outcomes that the program has defined other than (a) through (k) from EAC's Criterion 3, and
    • Complete a second Table 5.1, as specified in the CAC Self-Study Questionnaire.
  • Additional Guidance for CAC/TAC Joint Evaluations
    When a program name requires joint review by both the CAC and the TAC, the program should follow the TAC Self-Study Questionnaire. In addition, the program should:
    • Complete Section 3.D from the CAC Self-Study.
    • Complete a second Table 5.1, as specified in the CAC Self-Study Questionnaire.
